Tirzepatide and sleep apnea in Bali: how to read the AHI result

Two sleep trials measured breathing interruptions per hour. Compare the results with and without baseline PAP use and see which number to bring to a Bali sleep appointment.

Week-52 mean AHI reduction: 25.3 versus 5.3 events per hour in trial one, 29.3 versus 5.5 in trial two.
Source: SURMOUNT-OSA investigators, 2024 (PMID 38912654). Bars show the magnitude of a reduction.

The headline result from SURMOUNT-OSA was a change in breathing events per hour of sleep. That matters if you are looking up tirzepatide for sleep apnea in Bali. A quieter night in a villa, a lower weight on the scale and a change in a sleep-study report describe three different things. Only one is the main measure that the researchers set out to test.

What the two sleep trials measured

Both trials enrolled adults with obesity and moderate-to-severe obstructive sleep apnea. In the first, participants were unable or unwilling to use positive airway pressure (PAP). In the second, they were already using PAP and planned to continue it. Researchers randomized participants to tirzepatide or placebo within each trial and compared changes over 52 weeks. The SURMOUNT-OSA investigators published the two results separately.

The apnea–hypopnea index, usually abbreviated AHI, counts pauses and partial interruptions of breathing per hour of sleep. Its unit has a denominator. Twenty fewer events per hour does not mean that a participant had only twenty events left over an entire night. To know the person's final result, you also need the starting value and a measurement at follow-up.

The 52-week results side by side

Trial populationMean change with tirzepatideMean change with placeboEstimated difference
Unable or unwilling to use PAP−25.3 events/hour−5.3 events/hour−20.0 events/hour
Using PAP at entry−29.3 events/hour−5.5 events/hour−23.8 events/hour

Read each row from left to right. In the first trial, for example, the average fell in both groups. Subtracting the placebo change leaves the published difference of twenty events per hour. The baseline mean AHI in that trial was 51.5. Combining that starting average with an individual traveller's current AHI would produce a number with no clinical meaning.

It is tempting to compare the two values in the right-hand column and conclude that PAP makes the medicine more effective. The trials did not randomize people between the two PAP situations. People entered each trial through different eligibility criteria. The comparison that the design supports is between treatments within a row.

If you already use CPAP in Bali

A CPAP machine report may show residual events while you are wearing the mask. A diagnostic report describes how your breathing was assessed under the conditions of that sleep study. Write those conditions beside each result. A machine screenshot from last night and a diagnostic study from last year are useful to a sleep clinician for different reasons.

For an appointment in Denpasar or elsewhere on the island, bring the original sleep report, the current machine report if available, information about nights of use, and your medication list. If your records are held overseas, a PDF with the test date, scoring units and interpretation travels better than a cropped chart. A clinician who sees all of that can decide whether reassessment is worthwhile. Continue prescribed PAP unless your treating clinician changes the plan.

What a local pen listing can answer

The Theoz tirzepatide product page identifies the local presentations by their labelled total contents. Keep that figure with the product paperwork. It cannot tell you your AHI, establish that your device is the trial product, or provide a reason to change sleep treatment. The trial also collected adverse events, most commonly gastrointestinal symptoms. Discuss those considerations alongside the potential benefit if a clinician is considering therapy.

A practical question to take away is, “Which measurement would you repeat to judge whether my sleep apnea has improved?” That makes the next conversation about your own breathing, treatment and follow-up. It also gives you a way to distinguish a meaningful change in your sleep record from a promising headline that describes somebody else's study.

Can a CPAP app tell me the trial result applies to me?

A device app can provide useful information about nights of use and events the machine estimates while it is running. The trials assessed participants under specified research conditions. If your question is whether the underlying condition has changed, ask the sleep clinician what measurement would answer it and whether it should be taken while using your current treatment. Keep the original diagnostic report available for comparison.
